What is HeartStart?

HeartStart is an entirely web driven program delivered at a fraction of the cost of Sigma’s full, on-site screening process. HeartStart was designed as a way for prospective patients to experience the unique laboratory analytics and same clinical interaction without participating in one of Sigma’s department-wide cardio-metabolic screening programs. It can also be used as a stand-alone cardio metabolic screening tool.

HeartStart, while not the complete Sigma screening package, is a simple and extremely cost-effective way of taking control of cardiac risk .

Cost

PROGRAM COST: $299 PER PATIENT

HSA/FSA Eligible

  • Includes phlebotomy visit at a patient service center
  • Sigma’s advanced lipid and bio-marker panel (identical to the testing used in nationally recognized on-site screening process)
  • 20 minute visit with a Sigma trained practitioner
  • 45 minute visit with a registered dietician
  • $200 credit towards Sigma’s flagship on-site screening process

How it Works

STEP 01

Schedule your blood draw at a local Patient Service Center

This will be scheduled through the Quest Diagnostics website and must be done after a fasting period of 8 hours.

Select “All Other Tests” and schedule your preferred location/date/time. Please ensure that the patient service center that you choose begins with “Quest Diagnostics.”

Under the insurance section, select “The person being tested doesn’t have insurance.”

STEP 02

Register for HeartStart and provide payment

Complete the HeartStart registration form and provide payment by clicking on the button below.

You will receive an email invite to create a Sigma Patient Portal account (be sure to check junk/spam accounts).

Use the patient portal to complete required patient forms, attend your tele-health visit with a Sigma practitioner, and access your patient file.

STEP 03

Schedule a tele-health appointment with a Sigma Licensed Practitioner

You will be notified once your lab results are available. Follow the prompts to schedule a follow up HeartStart appointment in your state.

Acknowledge prompts to confirm your tele-health appointment and complete required patient consent and intake forms prior to your scheduled appointment. These forms are located under the “Forms” section in your patient portal.

You will receive a link to attend your tele-health appointment. You can also attend your tele-health appointment by logging into your patient portal and locating your appointment under the “Profile” and “Appointment” icons.

It is recommended that you use Google Chrome web browser and a device with an updated camera and microphone for your tele-health appointment.

STEP 04

Register for HeartStart and provide payment

Complete this step after attending your HeartStart follow up visit.

This appointment will be made through the Quest Diagnostics 4MyHeart Program. Program details and the number to call to schedule your appointment can be found by downloading the brochure below.

Use the patient portal to complete required patient forms, attend your tele-health visit with a Sigma practitioner, and access your patient file.
